The stupidest thing I've ever seen anyone do or say in relation to a phone:

At a major New York City hospital, both the PBX and Centrex systems were in the same equipment room, next to each other.  Above the equipment room, on the roof, was a large water tank used for air conditioning and flushing toilets. If that tank ever ruptured, the water would destroy the equipment room.
